General comments
hartoperaties: profylaxe peroperatief te herhalen bij > 1500 ml bloedverlies of wanneer ingreep > 4 uur duurt (c.q. langer dan 2 x de halfwaardetijd van het betreffende antibioticum).
In het geval van cefazoline: vervolgdosis van 1 gram indien de verwachte resterende operatieduur 3 uur of minder is; is deze verwachting er niet, een vervolgdosis van wederom 2 gram cefazoline.
hartoperaties: cefazoline (of vancomycine/clindamycine) iv tot maximaal 24 uur postoperatief. Dat betekent 2 g cefazoline preoperatief, gevolgd door 1 g na 8 uur en 1 g na 16 uur.
hartoperaties en implantatie pacemaker/ICD: bij bekende S. aureus-dragers wordt aangeraden voor te behandelen met mupirocine-neuszalf.
bij type I allergie voor β-lactams: clindamycine of vancomycine, uit te breiden met gramnegatieve dekking bij hoge incidentie van wondinfecties met gramnegatieven. Voor adviezen bij langere operatieduur: zie SWAB richtlijn.
